If you are a new client and would like to schedule an appointment, please click on the “New Client Form” button below to fill out the client questionnaire. This should take 5-15 minutes, depending on how detailed you are. You can also call or email me directly.


I will contact you via email or phone, depending on your preference. We can have a 15 minute phone consult, or we can go ahead and set up an initial appointment.

Payments & Insurance Information

I accept private pay only. Session fees are $160 per 55 minutes session, and $85 per 30 minute session, with a $5 increase on January 1st of each year.


Sliding scale rates are not available at this time, and payment in full is expected at the time of service.


Your plan may have “out of network” benefits which might assist you in paying for treatment, and I can provide documentation for you if this is your situation. Check with your insurance company about this if you are interested! 


Good Faith Estimate Statement:

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your healthcare provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.


You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Your Good Faith Estimate will be provided to you in your intake paperwork. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.

Cancellation and

No-Show Policy

If you need to cancel an appointment, please do so at least 24 hours in advance of your scheduled appointment time. If you cancel an appointment with less than a 24-hour notice or do not show for your appointment, you will be charged $125 to the credit card on file for a scheduled hour appointment, or $55 for a scheduled half hour appointment.
